Sanjeev Bhatia M.D. Dr. Sanjeev Bhatia finished his residency in internal medicine in 1998 at NYU Downtown Hospital in New York City. He did his fellowship in Pulmonary and Critical Care from 1998-1999 at the University of Connecticut. Dr. Bhatia worked as an internist and ER doctor in the Midwest for five years before he moved to Victoria in 2004. Since 2004, he has worked as an internist and sees patients in the local hospitals, nursing homes, and assisted living facilities. He is board certified in Internal Medicine, Hospice and Palliatative Care & Sleep Medicine. In 2007, Dr. Bhatia established Crossroads Health Center. He also currently is a Medical Director at Victoria Warm Springs, Carter Home Health, LaBahia Nursing Home, and Hospice of South Texas. His special interests include complex medical disorders, sleep disorders, geriatric, and end of life care. He admits patients to all local hospitals and nursing homes. Dr. Bhatia is a firm believer in education and provides several scholarships in nursing and general studies including Athlete of the Year/MVP award for East/West High School which recognizes academics, as well as, community involvement excellence. |

John E. VanMetre M.D. John E. VanMetre, M. D. was raised in Iowa. He completed his undergraduate work at the University of Iowa with high distinctions and was awarded Phi Beta Kappa. He completed medical school at the University of Iowa and finished his residency in Family Medicine at University of Fargo North Dakota. He has been board certified in Family Medicine since 1991. Dr. VanMetre practiced in Iowa for 3 years and Nebraska for 17 years before coming to Victoria. In addition, he completed a National Health Scholarship assignment three years on the Cheyenne River Indian Reservation in South Dakota. Practice includes the gamut of non-obstetrical family practice. His outside interests includes: reading, games and puzzles, bicycling and piano. Dr. VanMetre is married with two children ages 21 and 14. |

Pamela (Pana) Breazeale FNP Pamela Breazeale is a certified Family Nurse Practitioner (FNP) with a long career in nursing. She started her nursing career in 1999 and had been employed by Citizens Medical Center as a charge nurse and also at the VA outpatient clinic in Victoria helping the men and women who have given so much of themselves for us by serving in the military. Pamela earned her Bachelor’s degree in Nursing through Texas Tech University in Lubbock and graduated with honors from Texas A & M in Corpus Christi with her Master’s in Nursing. Pamela started her FNP career at Crossroads Health Center in October of 2011. In addition to seeing patients in the clinic, Pamela also performs home visits to those patients that are homebound or otherwise unable to come into the clinic for health care. Pamela is committed to providing quality health care for all individuals and strives to ensure each patient receives focused attention. |

Kathryn (Kathy) M. Custer RN,FNP-C Kathryn Custer is a 2009 graduate of Texas A & M University at Corpus Christi, TX with a 4.0 GPA and member of the Honor Society of Nursing. She received her Bachelor of Science in Nursing at Texas Tech University Health Science Center in 2006 also with a GPA of 4.0 and a Suma Cum Laude graduate. In 1990 she received her associate of applied Science Degree in Nursing at Victoria College. Kathy has 22 years of nursing experience, accreditations and licensing which includes 10 years of charge nurse experience on the medical-surgical oncology urology unit and 5 years experience on the medical-surgical oncology urology unit and 5 years experience in home health. She is a highly competent practitioner with over 3 years of exceptional hands on experience as a Nurse Practitioner in acute and chronic management of patients. She also has extensive training in the diagnosing and treatment of chronic diseases such as diabetes, hypertension, hyperlipidemia, as well as acute patient problems such as upper respiratory problems, urinary tract infection and abscesses to name just a few. Kathy is extremely qualified in analyzing and interpreting patient histories, symptoms, physical findings and diagnostic information to develop appropriate diagnosis and treatment plans which makes her an excellent asset to Crossroads Health Center. |

Johnny M. Hasserodt FNP Johnny Hasserodt is a Family Nurse Practitioner that joined Crossroads Health Center in 2011. He has extensive experience in emergency and critical care bringing 20 years of experience to the clinic. He has worked in trauma centers in the Houston Medical Center gaining knowledge and skills for patients here in Victoria. He is certified as a Family Nurse Practitioner through the American Academy of Nurse Practitioners. He graduated from Texas A & M University-Corpus Christi where he was recognized and inducted into the Honor Society of Nursing International. He has degrees in nursing from University of Houston-Victoria and the Victoria College. He also is a certified Paramedic. In addition to providing care in the clinic he also has privileges in all the local hospitals and nursing homes under the direction of Dr. Bhatia. He believes in providing quality patient care all while focusing on the patient as a whole individual, bringing together their background, beliefs, race, disease processes, educational background and spirituality into consideration for developing a plan of care that will not only meet patients medical needs, but make them feel part of the care. His special interests include cardiovascular disease and the management of acute and chronic illnesses. |

Florence Oladoyin FNP Florence Oladoyin is a certified family nurse practitioner with more than 25 years of experience as a registered nurse in critical care, geriatric, obstetric, pediatric and urgent care settings. She graduated nursing school in 1983, earned a Bachelor’s degree in 1992 from Texas Tech University in Lubbock and a Masters degree in Nursing with honors from Texas A & M University Corpus Christi in 2011.
Florence worked as a charge nurse in Spohn Kleberg Memorial Hospital in Kingsville, Incarnate Word Hospital in Lubbock, Detar and Kindred Hospitals in Victoria. She joined Crossroads Health Center in 2011 providing care to patients with various health care needs. In addition to working in the clinic, Florence has medical care privileges in all local hospitals and long-term facilities, including nursing homes under the supervision of Dr. Bhatia.
